This evaluation is to be completed by the community co-educator with regard to a student’s work.  Questions are both quantitative and qualitative in nature and address student’s skill and performance areas.

This evaluation is to be completed by the community co-educator with regard to a faculty member’s involvement with the partnership.  Questions are based on a strongly disagree to strongly agree scale that address development, communication and overall experience.
